At this time, Live Well is a fee for service practice. Full payment is required in full at the time of service. If you would like to use your insurance, as many clients do, we can provide you with a superbill (receipt) so that you may send in self-claims for out-of-service reimbursements.

At this time, Live Well is not a Medicare Provider, therefore, we cannot legally provide services that would be considered normally covered services according to Medicare. If you are seeking Vestibular Rehabilitation, we are happy to refer you to a medicare enrolled provided.
